remove spare tire?

on my 03 dodge ram van. tire is under van, plate holding tire on, but how do i get plate off, weird shaft coming out ,not sure how to turn it? thanks in advance.

The shaft coming out (usually to a hole in the back bumper) should match the tapered end of your lug wrench. Insert the lug wrench and it becomes a crank to lower the spare.
